Riccarton Clinic, supported by Christchurch Primary Health Organisation, provides an outreach nursing service targeting under serviced high risk patients in addition to routine general practice services. The Outreach Nurse may be referred to you by your doctor.
The goal of the community outreach registered nurse is to successfully connect families and individuals to Riccarton Clinic’s programs and existing community resources to achieve their optimal health potential.
Community outreach registered nurses coordinate disease specific screenings, such as breast cancer, and cervical cancer screening each year.
The desire of each community outreach registered nurse is to empower all members of our community to enjoy the healthiest lifestyle possible.
The Outreach Nursing Service is an extension of our own General Practice and support us to do:
Cardiovascular Risk Assessments in the community
Diabetes Annual Reviews in the community
Immunisation recalls
Cervical screening recalls
Long Term Conditions Assessments and management in the community
Outreach nurses can meet with you at a location that is convenient to you, for the following:
Health assessment which can identify your health needs
Provide health information to enable informed choices
Facilitate access to appropriate health providers
Provide support with specialist/hospital appointments
Self management skills
Smoking cessation
Accessing GPs appropriately
Medication Management
Assistance with reducing barriers to accessing health care
Cost
Transport
Use of Interpreters
